What is a Deprogrammer?

To a dentist, this is not someone who rescues someone from a cult. It is a device, similar to a Hawley retainer with a small palatal bite plane, that when worn, opens a patients bite and allows only a single point of contact just behind their maxillary central incisors. Deprogrammers are advocated by some dentists as a means of making the muscles involved with closing to relax so that a patient will be able to find their Centric Relation (CR) position more easily. Some patients with inflammation of the muscles involved with closure have difficulty closing in a reproducable CR position, but after wearing a deprogrammer for a week or It is use two, they experience less muscular inflammation and are able to be easily guided into a CR position. Thus the device is said to "deprogram" the muscles involved with closing and is often employed with patients who are experiencing TMJ pain.

After wearing one of these devices for twenty three hours a day the muscles involved with closing will in theory become more relaxed and allow a patient to more easily close their mouth in centric relation. Deprogrammers have a single point of anterior contact tends allow over used and inflammed  masseter muscles to calm down.  Centric Relation is a term dentists use for" the most retruded position a patient can comfortably close their mouth"and is employed by dentists as a basis for performing any full mouth reconstruction involving a denture or many crowns or bridges. In this supposedly reproducable position, the condyles are centered and positioned properly in the glenoid process of the skull's temporal bone.

 Paradoxically, Deprogrammers can sometimes cause TMJ pain to worsen, especially if a patient has inflammation in the posterior portion of the capsular ligament, since these devices tend to put more pressure on this part of the capsule. If after wearing one of these devices a patient's pain worsens, they should  remove the device and contact their dentists for further instructions.

    Hi Dr. Spindel!
    I have a questions in regards to Ortho vs Appliances for TMJ problems. I am 34yrs old and I have severe facial, jaw and mouth pain. My jaw pops and my right condyle is worse then the left. I suffer from daily headaches and migraines atleast once a month. I have tried appliances, ortho and composites to open the bite (I have a very deep bite) but treatment was never complete due to associate changes in the office. My question is (and I know you must have an evaluation first but from your professional experience) what treatment is better to correct a deep bite, 2nd molars that do not touch and severe pain? One doc told me to ortho and another said daytime/night appliances.

  2. Unless you have damage in your posterior capsule, a deprogrammer or any night guard that prevents bruxing movements may help. Some patients, who use an anterior loading device can experience pain in their joint. A good way to test whether you are one of those patients is to place a cotton roll in between your front teeth and bite down gently. Hold it gently for 10 minutes and if it causes pain, than an anterior loading device will not make your jaw feel better. (this is my interpretation of a Frank Spear lecture on occlusion)

    I am 49 and have had some muscle tension and soreness in my lower jaw.I have also had some wearing on my front upper and lower teeth and uneven bite. Mt Ortho put me in a deprogrammer for 3 weeks.It has moved my teeth and bite.I have discontinued use and would like to know if my bite and teeth move back? if so,what does this mean and what are the further treatments?? If my teeh and bite do not move back what does this mean and what are the best treatments?

  4. Using a anterior deprogrammer will not usually alter the position of the teeth in that short period of time. What it can do is alter the level of activity of the muscles responsible for positioning the mandible and make them more relaxed. Unfortunately this can result in a patient experiencing a different bite where their teeth do not meet properly anymore. Time will tell if this will get better, but probably you should not use your deprogrammer anymore.

  5. I have tmj I’ve tried acrylic night guard and can’t fall asleep with them! I grind my back teeth while sleep but I also get headaches but rarely but I do get facial pain and ear pain my jack cracks pops and gets stuck open or closed would this help me?
